? Service Background
Where there are hazardous factors of occupational diseases listed in the catalogue of occupational diseases in the workplace of an employing unit (except coal mines), the hazardous items shall be declared to the local administrative department in a timely and truthful manner and shall be subject to supervision and management. When the employing unit declares an occupational disease hazard item, it shall submit the Occupational Disease Hazard Project Declaration Form and the following documents and information:
(1)The basic situation of the employer;
(2)Types, distribution and number of occupational hazards in the workplace;
(3)Other documents and materials as stipulated in laws, regulations and regulations.
? Service Content
If the employing unit has one of the following circumstances, it shall, in accordance with the provisions of this article, declare to the original reporting authority the contents of the project for the change of occupational disease hazards:
(1)For new construction, alteration, expansion, technological transformation or technology introduction construction projects, the declaration shall be made within 30 days from the date of completion and acceptance of the construction project;
(2)If the original declared occupational disease hazard factors and their related contents change significantly due to changes in technology, technology, equipment or materials, the declaration shall be made within 15 days from the date of the change;
(3)If the employer's workplace, name, legal representative or principal person in charge changes, the declaration shall be made within 15 days from the date of the change;
(4)After the detection and evaluation of occupational disease hazard factors, if the original declaration content is found to have changed, the declaration shall be made within 15 days from the date of receipt of the relevant testing and evaluation results.
